Terms & Conditions for Virtual Training Sessions 

By registering for a Virtual Training Session (VTS) with Citywide CPR, Inc, you (the participant) accept and agree to be bound by the terms and provisions of this agreement. In addition, when using these services, you shall be subject to any posted guidelines or rules applicable. Any participation or registration will constitute acceptance of this agreement. 

Reschedules and Cancellations 

You may cancel your registration at any time prior to your scheduled Virtual Training Session. Contact customer service via email, info@citywidecpr.com with your request and explanation. If no equipment has been shipped, you may receive a refund less a 10% administrative fee. If equipment has been shipped, shipping costs will be deducted from the refund, in addition to the 10% administrative fee. The refund will not be processed until the returned equipment is received. No refunds will be permitted for cancellations within 24 hours of a scheduled skills session, or for missed skills sessions. In this case, you must reschedule and pay a rescheduling fee of $20.00, or forfeit your payment and return the equipment. 

You must log into your Virtual Training Session on time. The Google Meet link will go live up to 10 minutes prior to the scheduled meeting. The instructor reserves a full hour for your skills session. If you are more than 15 minutes late, or request to reschedule within 24 hours of the scheduled skills session, you must pay a $20.00 rescheduling fee and reschedule with the assigned instructor. 

You must prepare for your Virtual Training Session by reviewing the equipment checklist, setting up required equipment, and completing any necessary precourse work. If you are unable to successfully pass your Virtual Training Session due to inadequate preparation, you will be required to pay a $20.00 rescheduling fee and asked to schedule another skills session with your instructor once you feel adequately prepared to reattempt your skills session. 

Shipping Equipment 

Once a completion certificate is received an instructor will be assigned and will reach out to you within 24 hours to schedule your one-on-one skills session. Citywide CPR, Inc agrees to ship the equipment necessary to perform the registered Virtual Training Session. Shipping will take 2-5 business days for delivery once initiated. You will receive a shipping notification including a tracking number for UPS. It is your responsibility to track the expected delivery date of the equipment and make necessary arrangements to secure the equipment timely. Citywide CPR, Inc does not bear responsibility for lost or stolen equipment once the equipment has been delivered to the address provided, as confirmed by UPS Tracking. Consequently, there is a $100.00 Equipment Restocking Fee for equipment that is lost or stolen after delivery. Please notify customer service immediately to initiate a new equipment shipment.

Citywide CPR, Inc is not responsible for any shipping delays and will make every effort to deliver your equipment prior to the scheduled Virtual Training Session. If the equipment will not arrive in time, your skills session may be rescheduled. 

Equipment will be sanitized and checked prior to shipping. If there are any damages or malfunctioning equipment, you must notify customer service immediately. Arrangements will be made for the equipment to be returned and new equipment shipped, if necessary. If equipment is damaged due to the participant’s negligence or is not returned, the student will be charged the full retail price of a replacement item. 

Your American Heart Association certification (eCard) will not be processed until your equipment is dropped off and the prepaid return shipping label is scanned by UPS. Equipment must be returned within 72 hours following the completed skills session. Please be mindful that equipment is limited, and in order for Citywide CPR to ship equipment timely, it must be returned as soon as possible. Failure to return the equipment timely will result in a $20.00 penalty, to be paid prior to processing of the eCard. 

If the equipment is not returned within 14 days of the skills session, you must pay a Lost Equipment Fee of $200.00. Failure to pay the fee will result in the cancellation of your eCard and the account being pursued by our collections department. 

Requirements & Expectations 

All participants must have adequate and reliable internet in order to complete the Virtual Training Session. You must have a functioning camera that is on at all times during the Virtual Training Session. Citywide CPR, Inc is not responsible for the participant’s technical equipment functionality or internet availability. If there are technical difficulties during the skills session, you may be required to reschedule. 

You must have space to complete your skills session that is free of excessive noise or distractions. If you are unable to ensure a quiet environment, you may be asked to reschedule the session to another date or time. 

Code of Conduct 

It is expected that each party will behave professionally and respectfully. Offensive, inappropriate or abusive language or behavior toward the instructor will not be tolerated. Failure to do so will result in immediate termination of the skills session. Your registration will be escalated to the Public Class Administrator to determine the next course of action, which may include a rescheduling fee.

Safety 

Equipment will be sanitized prior to shipping. Although manikins come with lungs installed, students will not be required to perform mouth-to-mouth ventilation on the manikin at any point during the Virtual Training Session. Therefore, we requested that you do not place your mouth on either the adult or infant manikin, other than to inflate as necessary. Cleaning wipes will be included with your equipment for additional cleaning as necessary. 

Liability 

You waive your right to claim for any and all damages or injuries that may result from participation in the Virtual Training Session. You understand and acknowledge that there are inherent risks of injury involved in performing CPR/AED and First Aid skills. You agree to release Citywide CPR, Inc and any employees from any and all claims which may result from the course. 

American Heart Association Course Fees Disclaimer 

The American Heart Association strongly promotes knowledge and proficiency in all AHA courses and has developed instructional materials for this purpose. Use of these materials in an educational course does not represent course sponsorship by the AHA. Any fees charged for such a course, except for a portion of fees needed for AHA course materials, do not represent income to the AHA.
